We support clients recovering from workplace injuries through structured, evidence-based rehabilitation.

What we need before your first appointment:

Contact GSMC and our admin team will send you a screening referral before your initial appointment.
A current WorkCover claim number and insurer details
Contact for your case manager (if applicable)
Details of any approved sessions

What to expect:

You’ll receive an individualised plan that evolves as you recover, and we’ll stay in communication with your insurer and rehab team. We support return-to-work goals and gradual reintroduction to physical activity where appropriate.

TAC

Provide your TAC claim number and accident date
Share any approval letters or rehab contacts

NDIS

Contact GSMC and our admin team will send you a screening referral before your initial appointment.
Provide your NDIS number and plan dates
Let us know if you are self-managed, plan-managed, or NDIA-managed
Share relevant goals or plan sections
Provide contact for your support coordinator or plan manager

Please note:

If You’re a DVA Client

We support veterans through DVA-funded services focused on strength, movement, chronic condition management, and injury rehab.

Before you arrive:

Bring your Gold or White card
Ask your GP for a referral or care plan D904

If You’re a Carer or Support Worker

We welcome carers and support workers and ask that:

All clients requiring assistance attend with an approved capable carer
Carers remain with the client at all times during appointments or hydro sessions

GP CHRONIC CONDITION MANAGEMENT PLANS (GPCCMP)

A valid GPCCMP referral from your GP (with specified services)
Any relevant scans, reports, or letters

Medicare provides a rebate of approximately $61.80 per session (set by Medicare), not full coverage.
Full payment is required on the day, and we process your Medicare rebate immediately back to your account.
GPCCMP plans do not cover services such as hydrotherapy, massage, or recovery tools unless delivered as part of an eligible session by a registered provider.
